What should you ask before hiring a med spa marketing agency?

The short answer

Ask what the agency is actually measured on (leads or booked consultations), who follows up with leads and how fast, how they choose who sees your ads, what they track, what they need from you, and which named clients they can show results for. The answers tell you whether you’re buying leads or buying a fuller schedule.

Eight questions to ask on the sales call

1. “What are you measured on?”

If the answer is clicks, impressions or cost per lead, you’re buying leads. Look for an agency that reports on booked consultations, show rate and revenue.

2. “Who follows up with my leads, and how fast?”

This is where most campaigns quietly fail. If the answer is “your front desk,” ask what happens to a lead that comes in at 9 p.m. on a Saturday.

3. “How do you decide who sees my ads?”

Broad interest targeting reaches browsers. Ask whether they can target people who are actively researching your treatments and qualify them by ability to buy.

4. “What do you track, and how does booking data get back to the ad platforms?”

Without booking data, the platforms optimize for form fills. Good agencies set up conversion tracking tied to real appointments.

5. “Can you show me results from clinics like mine, with names?”

Screenshots with no clinic name aren’t proof. Ask for named clients, video testimonials, or results you could verify.

6. “What happens in the first 90 days?”

A clear plan should cover when ads go live, when they start optimizing, and what a results review looks like at the end.

7. “How do you help us get found on Google and in AI search?”

Ads stop when the budget stops. Ask how they build visibility that keeps working, including in ChatGPT and Google’s AI answers.

8. “What do you need from us?”

An honest agency will tell you. If they say “nothing,” be skeptical.

Red flags

  • Guaranteed rankings or a guaranteed number of patients for every clinic, regardless of market.
  • Reports that only show clicks, impressions and cost per lead.
  • No named client results.
  • They won’t give you access to your own ad account data.
  • Nobody on their side owns what happens after a lead comes in.
